What the $#@! does that mean?
Cazzo means penis or c0ck in Italian. But they use it in a saying which doesn't translate well into English. They use it to describe something like "what the fuk!!? Or what the hell!!?" not really gonna say what the c0ck or penis now ;)

Why is it La Vecchia Signora and not La Signora Vecchia?
Some adjectives come before nouns, in this case they are fixed expressions.

vecchia signora
bella vita
curioso caso
vero disastro
etc

I suppose it's something you pick up with experience. Some are obvious - una grande pizza (a great pizza), una pizza grande (a large pizza). Things like that.
