I haven't been following this case much, but:

On April 12, 2015, Freddie Carlos Gray, Jr., a 25-year-old African-American man, was arrested by the Baltimore Police Department for possessing what the police alleged was an illegal switchblade.[2] While being transported in a police van, Gray fell into a coma and was taken to a trauma center.[3][4] Gray died on April 19, 2015; his death was ascribed to injuries to his spinal cord.[4] ...

... The medical investigation found that Gray had sustained the injuries while in transport.[8][9] ...

Clearly it's not okay for a person to die while being transported by the police. What was the explanation?

Unable to prove maybe. But the very fact this guy is dead proves it must have been there. People don't just die like that. We had a similar high profile case in Belgium btw. Some dude high on metamphetamines was arrested and they beat him, supposedly to contain him, only to have him die from the resulting injuries.

The problem is this guy is dead though. If you are responsible for someone's death, jail time should definitely be on the cards. Especially considering how easily people are effectively jailed in the US as a country.

Someone needs to be held accountable for the safety of people brought into custody, no question. Whether that constitutes criminal negligence is another matter, which is why the courts are involved.

Even in cases where the shootings are somewhat justified there was a mistake made that led to an escalation that resulted in a death. If a brain surgeon screws up they are a minimum sued, at maximum lose their license to practice medicine. Why should police avoid real world repercussions for their actions?

At a minimum, there maybe should be a point-person fall guy in charge of the safety of an apprehended person. If you don't make it someone's job, stuff doesn't happen or people are encouraged to be careless.
