Who was the last player suspended for diving?

A player normally gets a yellow for diving, that's what he should get, not worse because a ref missed it.

Players dive all the time - I have never seen a player get more than a yellow for diving - what are they going to start reviewing every game in detail for everything that looks like a dive?
