You'll have to go back to the time when not more than 2 teams from the same country were allowed into the CL. To be exact, in 2001-2002 only Juve and Roma qualified for the CL.
In serie A 2000-2001, Inter and Milan both finished outside top 4, so even if 4 Italian teams were allowed to CL, those two weren't going to make it.
