she trusted littlefinger and convinced her husband to trust him.
she was quick to believe petyr when he said the knife belonged to tyrion
she took tyrion hostage, causing more tension and conflict between starks and lannisters. ned already had suspicions that the murder of jon arryn was by the lannisters, when in reality he was indirectly murdered by petyr, cat's "little brother"

ned also made grave mistakes, biggest by far was telling cersei he knew about her and her brothers nightly activities
robb stark executed karstark amidst a war (losing a lot of his army) AND undermined frey, a very dumb move
robert was a drunk and a fool, never shouldve been king
petyr the conniving villian of the show who orchestrated most of the events that lead to the war
and of course, the lannister family, featuring the incest twins and that devil they called joffery

Cat believed and trusted her sister, can't really be blamed for that, robb lost the war when he didnt pursue the retreating lannister forces and for trusting their eternal rivals the boltons.
Also the whole love affair thing. Can't blame him though, she was fine.
