They might play against Portugal, but certainly not in the first play-off match.
The playoffs are played in 3 groups (A, B, and C). Each group will have two seeded and two unseeded teams. Portugal and Italy are both seeded and they might find themselves in the same group, but they could face eachother only in the group's final. In the group's semifinal, a seeded team will play against an unseeded team and the winner goes to the group's final against the winner of the other encounter in the same group.

Italy's potential opponents in the semifinal are: Austria, Czech Republic and/or Wales, Macedonia, Finland/Ukraine, Turkey/Norway, possibly Poland, and maybe even Sweden

Italy played like shit post the EUROs but for me Jorginho is the main culprit here for missing not one but two crucial penalties against Italy's direct rival in the group. He's awful at penalties with his stupid technique that got exposed several times in the past few months. He put the Italian team under huge pressure against a N. Ireland team that haven't conceded a single goal at home.

Immobile's importance to the overall Italian attacking phase is also quite clear. He's a reference point and his presence makes the wide men Chiesa and Insigne less predictable.

European World Cup qualifying playoffs will be different this time around: No two-legged playoffs. 12 teams into three seeded groups next March for single-elimination semis and final, 3 of the 12 teams make it to Qatar. Italy and Portugal will be in the playoffs, among others.

They did change the format, but it's still 2 play-off matches, as it used to be, just that now they won't play the same team home and away, but two different teams in a 1-leg match.

Worst case scenario for Italy is: Italy-Poland/Czech Republic/Austria in the semis, and then Portugal-Italy (in Lisbon) in the final.
Best case scenario: Italy-North Macedonia in the semis, and then Scotland (in Rome) in the final.

This playoff format makes my head hurt..

If you win the ‘final’ in your group. Are you through to the world cup? Or do you still need to play the final winners from the other groups?

There are (will be) 12 teams who'll make the play-offs - the 10 runner-ups from the 10 groups, and the 2 best Nation League group winners who failed to qualify via the groups (one is Austria, and the other one will be Czech Republic or Wales).

These 12 teams are divided into 6 seeded and 6 unseeded teams. 3 groups will be created, each consisting of 2 seeded and 2 unseeded teams.
In the group's semis, a seeded team plays at home against an unseeded team. The winners from both group matches will play in the group's final (the home team yet to be drawn). The winner makes it to the World Cup.

All three group winners will go to the World Cup.

As things stand right now, the seeded teams are: Portugal, Scotland, Italy, Russia, Sweden and Poland, with the Poles probably losing this position tomorrow to Wales. The unseeded teams are: Wales, North Macedonia, Turkey, Finland, Austria, Czech Republic.

I had no idea they had changed this. FIFA or UEFA love to overcomplicate the simple things (for profiteering purposes of course).

If you asked them to make a cup of coffee they would insist you watched them milk the cow and grow the beans. Not to mention they would make you watch the manufacturing process of the porcelain cup they are serving it in. Fucking leaches

Indeed. The draw will play a big part.
Italy might play North Macedonia or Finland in the semis and Scotland in the final in Rome. But they also might get a tricky opponent like the Czech Republic, Austria or Poland/Wales in the semis and then go to Lisbon or Stockholm to play Portugal or Sweden in the final.
