Well, at my university, you have to choose a main area (Europe, America, Asia/Africa, Global/Thematic), do most of your courses in that are and you have to also take a few courses in the other areas.
With the MA, you specialize, in a period for example Medieval Christendom or Ancient Rome. With the PhD, it's even more specialized as it is usually an event(the Crusades for example).
