with Juve I play mostly 4-4-1-1 or 4-4-2
Full backs making runs and crossing from deep
one very defensive DC and one to carry the ball out
2 DMs, one (Sissoko) who runs around and tackles, set to play defensively without a lot of forward runs with our without the ball. the other DM plays more like a CM, creating and shooting when possible
2 wingers (gio/deceglie and camo)
and then, depending on the guaranteed injuries, either giovinco behind a striker (amauri is a great lone striker) or two strikers.

I sometimes switch to 3 DC, 2 wingbacks, 2 CM, 1 AM, and striker if i have to.

what i've found out after a season:
keep the same formation when possible!!!!!! it really does make a difference, as the team will gel well after 5-8 matches. What really makes a difference, is the individual player instructions. i set them differently depending on how strong a team i'm facing, and it's also very important to set opposition instructions. it's usually a good idea to close down ALL AMs and Ms, with hard tackling and for central players, set to weaker foot. for strikers, i usually just set show to weaker foot. it can also be very helpful to close down fullbacks, especially against teams with strong attacking FBs. if you really want a pressing game, close down the CDs as well. tackling should be hard against everyone but the strikers.
some other hints:
your true defensive midfielder should probably have short passing
you do not want there to be too big a difference in mentality between any of the different lines.
obviously, if you're worried about getting buried by barca away, counter attack with quick direct passing, but occasionally switch to slow, short passes. I managed to get a 0-0 (after winning 1-0 at home) at camp nou with only Buffon, legro, molinaro, and an injured poulsen from the 1st team. i even had arena from the youth team starting as striker. complete miracle i'll admit!
