4th EPL team had to play in the qualifiers anyway. This "4 teams of top leagues get automatically into group stage" was implemented from 2017/18 only.

Yes, 4th team plays in the Play-off, it's one step before Group Stage. Yes, but before there were already 3 federations with 4 teams each on the Group Stage as well.
