Questions: 1- did Nedved and 11 come on the jersey or did you have to attach it yourself?

Yes Nedved 11 was on the jersey
and No the scudetto will not fall off if done properly.
The glue is pretty serious and I make sure it is ironed on for life lol.
A good 5 minutes for application, with the use of a towel protecting the scudetto and jersey from the irons direct heat.

I usually don't get personalized jerseys. Infact, my only personalize jersey is the Juventus 1995-96 CL final. And it is personalized Del Piero. It is XL so I don't ware it while playing football (I wear M) plus, I ain't playing with a jersey that expensive.. So that one is for special ocassions.

Do you have to wash the jersey before you iron the scudetto badge?

generally no
if you wash the jersey gently by hand with a little laundry detergent
then hang dry it
your jersey and scudetto will remain in good condition like new
