I don't think Israel will survive for more than 10 years from now. Iran is all talk but is only paperweight. In reality, its lost control over Syria, and Hizbollah and the Houthis have been significantly weakened.
The real danger for Israel is if another power emerges from within the region, after the Middle East destabilise. Borders will break and new nations will be formed.

I have a bad feeling about all of this, especially with war looming both in Europe and the Middle East but hopefully I'm wrong.

I can't imagine another power emerging. The ones that exist already seem to be happy to let Israel do what it wants as long as they continue to enjoy their wealth. Saudi Arabia and Qatar are traitors to their Muslim brothers. At least that's my perspective from a very limited knowledge of the regional politics.

Does the regime actually fall here? Does Israel succeed in its goal of regime change?

State failure is sufficient. All they need is for the regime to lose control in some areas.

What I expect is that Israel will retaliate even harder to Iran's retaliation. Israel eventually wins the escalation ladder on its own (since it can go nuclear and has showed it can do anything it wants) but they'll probably get the Americans directly involved to cause swift state failure.

Then the next push is on Egypt and Jordan to take the Palestinians which they'll have no choice but to say yes.
