what determined what group winners get the 3rd place teams for the round of 16? Doesn't seem fair if it was pre determined before the tournament.

Due to the "luck" of the draw Italy was screwed by getting in group E. E and F (because they are the last two groups) don't get the chance to play 3rd place teams. It's a terrible, disgraceful system but it is what it is.
