Just an idea, perhaps you tried to remove the scam advert using Adblock? Try with Adblock disabled (we don't have any ads anyway). Maybe that's not it but worth trying. Looking at that screenshot it appears it has problems loading the css (style) file.

OK now it should work for sure, I've studied the html5 video tag: http://www.w3schools.com/html/html5_video.asp So mp4 should work for all modern browsers but I've also added the webm fallback just in case. BTW there's no such thing as a gifv format, its just pointing to mp4 and webm files...
