Yes, they should've - if each of them were a single change in the regular squad. But there's no way for them to be ready to function as a unit having no prior experience in such a mode.
Good question. Allegri rotated the GK more than the CBs for the first half of the season. Even against Bologna for the cup we fielded Chiellini-Bonucci, instead of giving Benatia some minutes. By doing that Allegri killed two birds with one stone: he forced Benatia out and ran Chiellini and Bonucci to the ground. Masterfully done.
I like Caceres but really never been convinced of his defending. He has always seemed like someone who relies on his physicality than defensive knowledge. Which worked at RB/3man RCB, but could get exposed at CB in a 2.
His positioning is often wrong and he doesn't make the correct decision when knowing when to just stand his ground. Inglese had once chance in the second half when Caceres seemed determined to try to steal the ball, he couldn't and Inglese got the shot away. If he just stood his ground behind there'd be no issue.

As for Rugani, I'd like him to be sold to Chelsea during the last summer for those useful 60 millions with Caldara staying in Turin, but that's not how the club saw it, unfortunately.

Ruganis has his positives over Bonucci. the most important one, he wont get bullied in the air like Bonucci always does. but these last 2 games show that he has a long way to go before being a regular starter here.
