It's indeed more of a pain reliever. Besides if you want an ankle sprain to heal properly you're going to have to use anti-inflammatories.

Must be some specific sort of anti-inflammatories. I don't think most anti-inflammatories are on the list. Besides, they're used very often. Imagine a Milan player spraining his ankle, surely they're going to give him anti-inflammatories.
Cortisol (hydrocortisone) is a steroid type of hormone in the body, released when in pain or when stressed in order to help the body recover. It has similar effects as adrenaline, and helps us perform better when we need it the most, such as in an important game of football. Basically, it makes the player more euphoric and energetic.

Regularly, anti-inflammatories used in sports will not increase the cortisol levels of your blood significantly, if with anything at all.
